Terms of Payment by Invoice

This order is entered into by and between Mediphany Inc., a Delaware corporation (“Mediphany”), and the party identified as the “Customer” below. The Terms of Service located at this linked page are incorporated into and made a part of this order. Mediphany and Customer agree that the products and/or services described in this order will be provided in exchange for the fees described and pursuant to the terms and conditions of the Terms of Service. No terms or conditions of a Customer’s purchase order will add to or change any portion of this order or the Terms of Service and are hereby expressly disclaimed. This order is confidential and to be read only by the intended recipients. Capitalized terms used herein that are not otherwise defined will have the meaning attributed to them in the Terms of Service. By checking the associated checkbox, Customer represents that Customer has read and understands this order and agrees to be bound by the Terms of Service. A duly authorized representative of each party has executed this order and its governing Terms of Service as of the last date set forth below (the “Effective Date”). This order shall expire if it remains unexecuted by Customer more than thirty (30) days after the order listed above.
